New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure.

You will need to complete a pre registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
If you have a valid UK medical card, please bring it with you to the surgery to register. If you have lost your medical card or do not have one we will ask you to complete a further form.
All patients over the age of 5 will be offered a health check. All our children will be invited for immunisation, please attend the baby clinic.
Where patients are requesting to join the practice list, the practice does not discriminate on the grounds of race, gender, social class, religion, sexual orientation, appearance, disability or medical condition.
Medical treatment is available from the date of registration. Please contact reception for further information.

Change Of Address
If you change your address please complete the form which is available at reception in order that our records may be updated. Please bring in confirmation of your change of address.
You should also inform the hospital if you are waiting for an appointment.
Change of telephone number
Please advise us of your new telephone number as soon as possible
